About the Role

We are seeking a skilled and experienced Care Coordinator to join our team at Lewisham CMHT. As a key member of our multidisciplinary team, you will work closely with service users, carers, and other healthcare professionals to provide high-quality care and support.

Key Responsibilities
  • Develop and implement individualized care plans for service users with complex mental health needs
  • Work collaboratively with the multidisciplinary team to provide comprehensive care and support
  • Conduct risk assessments and develop strategies to mitigate risks
  • Provide education and support to service users and carers on mental health issues and treatment options
  • Participate in regular team meetings and contribute to the development of care plans and strategies
Requirements
  • Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse and/or Registered Occupational Therapist and/or Social Worker
  • Significant relevant clinical experience
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Knowledge of clinical governance and audit
