About the Role

We are seeking a skilled and compassionate Care Responder to join our Home to Decide Team in South Gloucestershire. As a Care Responder, you will play a vital role in supporting individuals in their own homes following a hospital stay, aiming to improve their quality of life and increase independence.

Key Responsibilities

  • Work with residents to support them at home, promoting independence and improving quality of life.
  • Develop and implement care and support plans in collaboration with practitioners.
  • Provide accurate records of visits in line with Council guidelines.
  • Work closely with practitioners to implement care and support plans, referring issues and discussing concerns as necessary.
  • Visit residents in their own homes, providing a response service 7 days a week between 7.00am – 10.00pm.

Requirements

  • Ability to show sensitivity and empathy when working with individuals with varying needs.
  • Effective communication skills, both verbally and in writing.
  • Hold a clean and valid driving licence and own transportation, with mileage expenses paid.
  • Relevant experience in the care sector and knowledge of relevant Care Act legislation and regulations.
  • Relevant vocational qualification, such as RCF - LEVEL 2, and/or CQC Care Certificate.
