We seek a compassionate, enthusiastic, and professional Community Nurse to join our dynamic and busy team. The ideal candidate will demonstrate a strong commitment to collaborative team efforts and innovative approaches to maintaining high standards of patient care. A genuine passion for community nursing and a commitment to ongoing professional growth are essential.
The ability to travel independently is a crucial requirement for this position.
Key Responsibilities
Understand and embody the Trust's vision, translating it into practical applications for daily operations within a community context. Support Caseload holders and Team Leaders in fostering the Trust's vision and cultivating a culture focused on patient-centered services. Provide nursing care to patients in their homes or preferred locations, offering alternatives to hospital stays, facilitating timely discharges, and empowering individuals to achieve their potential.We are committed to providing clinical support, continuous professional development, and training opportunities.
This role operates on a 7-day schedule from 8 am to 6 pm, managed through a Rota System.
