Job Title: Community Health Nurse Specialist

We are seeking a skilled and compassionate Community Health Nurse Specialist to join our team. As a Community Health Nurse Specialist, you will be responsible for delivering high-quality, patient-centered care to individuals in their own homes.

About the Role
  • Assess the physical and psycho-social needs of patients and develop personalized care plans to improve health outcomes.
  • Deliver therapeutic interventions based on best available evidence and work collaboratively with other healthcare professionals to ensure seamless care.
  • Manage a caseload of patients with complex health needs and provide regular updates to case managers and other stakeholders.
  • Participate in quality improvement initiatives and contribute to the development of clinical pathways and protocols.

Requirements
