Job Title: District Nurse/Community Case Manager Nurse

We are seeking a highly skilled and experienced District Nurse/Community Case Manager Nurse to join our team at Marlow Adult Community Health Care Team (ACHT). About the Role

The successful candidate will be responsible for providing high-quality nursing care to patients in the community, working closely with our multidisciplinary team to deliver effective case management and support. You will have a strong background in district nursing and community care, with excellent assessment, planning, and evaluation skills.

You will be responsible for:

  • Providing comprehensive nursing care to patients in the community, including assessment, planning, and evaluation of care needs.
  • Working closely with our multidisciplinary team to deliver effective case management and support.
  • Developing and implementing individualized care plans to meet the unique needs of each patient.
  • Collaborating with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Providing education and support to patients, carers, and families to promote health and well-being.

Requirements

To be successful in this role, you will need:

  • A degree in nursing or a related field.
  • Registration with the Nursing and Midwifery Council (NMC).
  • Experience of working in a community or district nursing setting.
  • Excellent assessment, planning, and evaluation skills.
  • Strong communication and interpersonal skills.
