District Nurse/Community Case Manager Nurse

We are seeking a highly skilled and experienced District Nurse/Community Case Manager Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Marlow Adult Community Health Care Team, you will be responsible for providing high-quality care to patients in the community setting.
Main Duties- Develop and implement case management plans to meet the needs of patients with complex health needs.
- Conduct comprehensive assessments of patients' physical and psycho-social needs, and provide evidence-based interventions to improve health outcomes.
- Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective communication.
- Manage a caseload of patients with long-term conditions, and provide support and guidance to patients and their families.
- Participate in the development and implementation of care pathways and protocols to improve patient care and outcomes.

Person Specification- Registered Allied Healthcare Professional / Registered Level 1.
- Nurse Degree/ Diploma in Health / Nursing Studies or related subject.
- Evidence of recent Continuous Professional Development.
- Relevant postgraduate clinical training e.g. Elderly, Long Term Conditions, Respiratory Disease, Cardiac Disease, Dementia, Rehabilitation, Trauma and Orthopaedics, and End of Life.
