Nursing Associate Role in Adult Community Healthcare

We are seeking a skilled Nursing Associate to join our Adult Community Healthcare Team. As a key member of our team, you will provide high-quality nursing care to patients in their homes and community settings.
The successful candidate will be part of a friendly and proactive team, working collaboratively to deliver excellent patient outcomes. You will have the opportunity to develop your skills and knowledge through our comprehensive preceptorship programme and ongoing support from our advanced community practitioners and practice development team.
Key responsibilities will include:
- Providing compassionate, safe, and effective care to patients in a range of care settings.
- Conducting ongoing assessments of patients, recognizing when to refer and escalate to a Registered Nurse for reassessment.
- Monitoring patients' conditions and health needs, working in partnership with patients, families, and carers.
- Recording all care delivered appropriately and contemporaneously.
- Communicating effectively with colleagues, providing clear verbal, digital, or written information and instructions when sharing information, delegating, or handing over responsibility for care.
- Recognizing and reporting any situations, behaviors, or errors that could result in poor care outcomes.

Person Specification- Registered Nursing Associate on the NMC register.
- GCSE Grade A-C/Level 4-9 in Maths & English or skills functional level 2 Maths & English or equivalent.
- Ability to take part in reflective practice and clinical supervision activities.
- Ability to communicate effectively (written, verbal, and non-verbal communication) with patients/relatives and carers and all members of the multidisciplinary team.
- Ability to develop effective and appropriate relationships with people, their families, carers, and colleagues.
- Ability to support, supervise, assess, and act as a role model to nursing associate students, other learners, and healthcare support workers as required within the clinical setting.
- Understanding of the scope of the role of the Nursing Associate in the context of the nursing and interdisciplinary team and the organization, and how the role may contribute to service development.
- Understands and acts in line with NMC professional standards for practice contained within The Code.
- Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice.
- Understand requirements for NMC professional Revalidation.
- Understanding of the importance of following procedures and treatment plans.
- Ability to organize and prioritize own delegated workload.
- Minimum 2 years' experience in healthcare.
- Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.
- Experience of providing and receiving complex, sensitive information.
