Job Title: District Nurse/Community Case Manager

We are seeking a highly skilled and experienced District Nurse/Community Case Manager to join our team at Marlow Adult Community Health Care Team (ACHT). This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our patients.

About the Role

The successful candidate will be responsible for providing high-quality nursing care to patients in the community, working closely with our multidisciplinary team to deliver comprehensive care plans. You will have the opportunity to work with patients with a range of complex needs, including those with long-term conditions, and will be expected to demonstrate specialist clinical assessment, analysis, planning, delivery, and evaluation skills.

Key Responsibilities
  • Provide high-quality nursing care to patients in the community, working closely with our multidisciplinary team to deliver comprehensive care plans.
  • Assess the physical and psycho-social needs of patients, instigating therapeutic treatments and care delivery based on best available evidence.
  • Develop and maintain partnership working with patients, carers, and other healthcare professionals to achieve optimal health outcomes.
  • Manage a caseload of patients with long-term conditions, working closely with our team to deliver coordinated care.
  • Contribute to the development and implementation of care plans, working closely with our team to ensure seamless care delivery.

Requirements

To be successful in this role, you will need:

  • A degree in Nursing or a related field.
  • Registration with the Nursing and Midwifery Council (NMC).
  • Experience of working in a community setting, preferably with patients with long-term conditions.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of modernisation agenda within the NHS and social care.
