Neonatal Community Outreach Nurse

Barts Health NHS Trust is seeking a highly skilled and experienced Neonatal Community Outreach Sister to join our team at the Royal London Hospital. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to neonates and their families, ensuring a seamless transition from hospital to home.
Main Responsibilities- Develop and implement community-based care plans for neonates, working closely with families, healthcare professionals, and other stakeholders.
- Provide education and support to families, empowering them to care for their babies effectively.
- Collaborate with the multidisciplinary team to ensure comprehensive care, including liaison with SLT, Dietician, and other healthcare professionals as needed.
- Participate in ward rounds and weekly MDT meetings to ensure effective communication and coordination of care.
- Monitor the quality and standard of care provided by the outreach team, completing monthly audits to ensure excellence in patient care.

Requirements- NMC registered
- Professional registration
- Teaching and mentoring qualification
- Specialist and intensive care experience
- Post-registration experience
- Experience at band 6
