About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Community Hub team. As a key member of our multidisciplinary team, you will play a vital role in providing timely support and assistance to clients who are struggling with their mental health and experiencing emotional distress.

Key Responsibilities
  • Conduct brief assessments to understand the client's current mental health status, risks, and determine appropriate interventions.
  • Provide psychoeducation to clients, offering them information and resources to help them better understand their mental health issues and develop coping strategies.
  • Work collaboratively with other community-based services to support service users with a range of mental health needs at varying stages of recovery.
  • Participate in team meetings, lead where necessary, and ensure effective handover of care to all staff involved in care.
  • Ensure that risk is systematically assessed and managed in accordance with Trust Policy.
  • Adhere to the Mental Health Act 2007 and the NHS and Community Care Act 1990.
Person Specification

We are looking for a candidate with:

  • Professional Qualification at Diploma or Degree level in mental health nursing, social work, or occupational therapy.
  • Demonstrable post-basic experience in Mental Health.
  • Experience of working in the community.
  • Understanding and promotion of equality, diversity, and human rights.
