About the Role

We are seeking a skilled and compassionate Mental Health Professional to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our rehabilitation and recovery services, you will play a vital role in supporting adults with complex and enduring mental health difficulties.

Main Responsibilities
  • Conduct clinical handovers and allocate patient care and therapeutic activities
  • Ensure all activities of daily living are supported in a person-centred way, focusing on independence and wellbeing
  • Administer medication and participate in care planning, risk assessments, and discharge planning
  • Supervise junior colleagues and contribute to multidisciplinary team meetings
  • Ensure the Mental Health Act is correctly applied and maintain accurate records
