We are seeking motivated and dynamic individuals to join our team as PCN Patient Support Specialists. This role is essential in facilitating the effective administration and execution of enhanced healthcare services across our network.
Key Responsibilities
The primary responsibilities of the position include, but are not limited to:
- Assisting in the implementation of enhanced healthcare services throughout the network, focusing on areas such as chronic disease management and mental health support.
- Contributing to the achievement of quality indicators, including immunization rates, cancer screening, and respiratory health management.
- Engaging with patients to address inquiries and requests through various communication channels.
- Promoting optimal use of appointment slots and access to enhanced healthcare services.
- Aiding healthcare staff in scheduling and coordinating patient appointments.
- Utilizing various IT platforms, including Microsoft Office Suite (Outlook, Excel, Word), EMIS Web, and AccuRx for efficient operations.
- Providing administrative assistance for network projects and other tasks that enhance the sustainability and resilience of our healthcare practices.

Candidate Profile
Required Experience
- Proven experience in a primary care setting.
- Familiarity with EMIS clinical systems.
- Proficient in using computer systems.
Preferred Experience
- Experience with AccuRx.
- Background in a general practice environment.
