About the Role

The Greenwich PCN Alliance is seeking a highly skilled Pharmacy Technician to join our team as a Medicines Optimisation Specialist. This is an exciting opportunity to work in a dynamic and fast-paced environment, contributing to the safe and effective management of medicines across our PCN practices.

Main Responsibilities

The successful candidate will work under the supervision of a Clinical Pharmacist and as an integrated part of the primary care team to ensure the safe, accurate, and timely supply of prescribed medication to patients. Key responsibilities include:

1. Repeat Prescribing and Repeat Dispensing Processes

a. Implement the PCN repeat prescription process, including repeat intervals for long-term conditions and complex drugs.

b. Agree a process for appliance requests, including stoma, continence, dressings, and ACBS products.

c. Support staff training and work with community pharmacy on electronic repeat dispensing (eRD) processes.

2. Patient Safety

a. Run searches and action MHRA CAS alerts to ensure patient safety.

b. Support patient queries related to medicines and refer complex queries to pharmacists, nurses, or GPs as needed.

3. Quality Improvement Projects and Clinical Audits

a. Identify patients suitable for virtual clinics to support QIPP implementation.

b. Identify patients who have not been issued anticoagulant, lithium, steroid, or antiplatelet cards and ensure read codes are updated in patient notes.

4. Health Innovation Network (HIN) Project BP@home, Lipid Management

a. Identify patients and risk stratify using UCLP search templates on EMIS.

b. Contact patients for up to date home blood pressure readings if available.

The Greenwich PCN Alliance is a collaborative partnership of four PCNs across Greenwich, Blackheath, and Charlton, Eltham, Heritage, and Unity.
