Job Summary

We are seeking a dedicated and compassionate Support Practitioner to join our team at Turning Point Scotland's Glasgow Mental Health & Huntington's Service. As a Support Practitioner, you will work with individuals who have a wide range of support needs, providing them with practical and emotional support and encouraging them to achieve their own personal outcomes in all aspects of their daily lives.

Key Responsibilities

  • Provide individualized support to adults with complex and enduring mental health problems, and individuals who suffer from symptomatic Huntington's disease
  • Assist individuals to maintain their own tenancies and provide support to help them achieve their personal outcomes
  • Work with individuals to develop and implement personalized support plans, taking into account their unique needs and goals
  • Provide housing support, personal care, and care at home packages as well as involvement in a range of education, employability & social activities through self-directed support
  • Collaborate with other health and social work professionals, advocacy services, and family and friends to ensure a comprehensive support network for individuals

Requirements

  • No previous working experience is needed; full training will be provided
  • IT skills are required for all our vacancies
  • Successful candidates will be required to register with the SSSC within 3 months of start date
