About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you will play a vital role in providing care and support to individuals with disabilities and additional support needs.

Key Responsibilities
  • Support individuals with daily living tasks, such as shopping, finances, cooking, and personal care.
  • Encourage and support individuals to engage in hobbies and interests, such as arts and crafts, gardening, and social activities.
  • Work collaboratively with the support team to ensure high-quality care and support is provided.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
Requirements
  • A caring and compassionate attitude.
  • Active listening skills.
  • Great communication skills.
  • A positive approach to learning.
  • Confidence in the use of computers.
