About the RoleWe are seeking a Support Practitioner to join our team at TouchBase Ayrshire, a welcoming and accessible environment that offers specialized services for disabled children, young people, and adults.
As a Support Practitioner, you will be responsible for providing tailored support to meet each person's needs, working closely with them, their families, and support staff to ensure their well-being and happiness.
You will be part of a dynamic team that provides support for communication needs, physical and learning disabilities, healthcare needs, and all aspects of personal care.
Key Responsibilities- Provide person-centered support to individuals with additional support needs
- Work collaboratively with colleagues and external agencies to ensure seamless support
- Develop and maintain relationships with individuals, their families, and support staff
- Contribute to the development of individual support plans and reviews
- Participate in training and development opportunities to enhance skills and knowledge
