About the Role

We are seeking an experienced Health and Social Care professional to join our team as a Lead Practitioner. As a key member of our team, you will be responsible for leading and inspiring a team of Support Practitioners to deliver high-quality care and support to our service users.

The successful candidate will have a strong background in Health and Social Care, with experience of leading and managing teams. You will be a passionate advocate for person-centred care and will have a sound knowledge of SSSC Codes of Practice and Health and Social Care Standards.

Key Responsibilities
  • Lead and manage a team of Support Practitioners to deliver high-quality care and support to service users.
  • Develop and implement effective service plans to meet the needs of service users.
  • Contribute to the development of policies and procedures to ensure compliance with regulatory requirements.
  • Work collaboratively with other teams to ensure seamless service delivery.
  • Provide guidance and support to team members to ensure they are equipped to deliver high-quality care and support.
Requirements
  • Strong background in Health and Social Care, with experience of leading and managing teams.
  • Passion for person-centred care and a sound knowledge of SSSC Codes of Practice and Health and Social Care Standards.
  • Ability to lead and manage a team to deliver high-quality care and support.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with other teams to ensure seamless service delivery.
