About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone. As a Support Practitioner, you will provide personalised support to individuals with complex needs, enabling them to live as independently as possible and participate in their local communities.

Key Responsibilities

  • Provide flexible and tailored support to individuals with complex needs, including daily living tasks, medication management, and financial assistance.
  • Support individuals to develop and maintain their hobbies and interests, promoting social interaction and community involvement.
  • Work collaboratively with the team to ensure a warm and welcoming environment, promoting a positive and inclusive culture.
  • Contribute to the development and implementation of individualised support plans, ensuring that the needs and preferences of each individual are met.
  • Participate in training and development opportunities to enhance your skills and knowledge, including the SVQ in Social Services and Healthcare.
