About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone. As a Support Practitioner, you will play a vital role in providing personalized support to individuals with complex needs, enabling them to live independently and participate fully in their local communities.

Key Responsibilities

  • Provide flexible and personalized support to individuals with complex needs, including cooking, meal planning, household tasks, medication, finances, appointments, and personal care.
  • Support individuals with their hobbies and interests, promoting their independence and well-being.
  • Work collaboratively with the team to ensure a warm, friendly, and supportive environment for all individuals.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.

Shift Patterns

We operate a 24/7 service, and shift patterns will include mornings, days, evenings, weekends, public holidays, and waking nights. You will receive your normal hourly rate plus an additional £20 for each night worked.

  • Confidence in the use of computers.
