Position Overview

An exceptional opportunity is available for a dedicated Patient Care Coordinator to become a vital member of the Medics PCN team. The Medics Primary Care Network is a collaborative of five supportive practices committed to delivering outstanding healthcare services to a diverse population.

As a key player in a growing multi-disciplinary team (MDT), you will engage closely with colleagues to ensure a seamless, professional, and adaptable experience for our patients. Your role will involve a comprehensive approach to integrating care and support tailored to individual patient needs.

You will assist the Medics Integrated Team in orchestrating essential activities, including service access, guidance, and information dissemination, ensuring that health and care planning is timely, effective, and centered around the patient.

The responsibilities include guiding and facilitating the patient’s journey through primary care. A significant aspect of your role will involve collaborating with the Lead Care Coordinator to support various components of the PCN deliverables, such as cardiovascular disease prevention and management, enhanced health initiatives in care facilities, and other health-oriented projects.

The ideal candidate will possess excellent communication skills and the ability to establish effective monitoring and quality assurance systems to track progress and outcomes. Proficiency in IT is essential to manage the administrative tasks associated with this position.

Main Responsibilities

  • Collaborate with the Medics PCN Integrated team and other primary care professionals to proactively identify and engage with a specific group of patients to provide personalized care.
  • Work in partnership with Social Prescribers, Health & Wellbeing Coaches, and fellow PCN Care Coordinators to explore care options that align with patient preferences.
  • Assist patients in utilizing decision-making tools to prepare for shared decision-making discussions and to develop individualized care and support plans in accordance with best practices.
  • Support patients in managing their needs by addressing inquiries, scheduling and managing appointments, and ensuring they receive high-quality information to facilitate informed choices regarding their care.
  • Help patients understand their knowledge, skills, and confidence levels when engaging with their health and wellbeing, utilizing tools such as the Patient Activation Measure.
  • Contribute to the PCN’s efforts in delivering the DES specifications.

About Us

The Medics Primary Care Network encompasses five practices, fostering long-standing relationships among member practices. Our network serves a population of approximately 59,000 patients and includes various specialist roles, such as Clinical Pharmacists, First Contact Physiotherapists, Paramedics, Social Prescribers, Health & Wellbeing Coaches, and Nursing Associates. Joining our team means becoming part of a vibrant MDT.

The addition of a Care Coordinator will enhance our dynamic team and support the proactive intervention programs initiated by the Medics PCN core team.

Qualifications and Skills

Essential:

  • NVQ Level 3 or equivalent Level 3 qualifications.
  • Demonstrated commitment to professional and personal development.
  • Proficient in Microsoft Office applications.
  • Experience in data collection and monitoring.
  • Familiarity with Electronic Patient Records.
  • Understanding of departmental policies and care pathways.

Desirable:

  • Qualification to Level 5.
  • Experience with IT systems, including SystmOne.
  • Background in community development, adult health and social care, or public health.
  • Experience supporting individuals and their families in a related capacity.
  • Experience in collaborative working across various organizations.
  • Experience in patient-facing roles.
