Job Summary

eQuality Primary Care Network is seeking a dedicated Clinical Care Coordinator to join our team. The ideal candidate will be responsible for coordinating patient care, identifying support needs, and providing navigation for patients and their carers.

Main Responsibilities

  • Proactively identify and support individuals with complex needs, providing coordination and appointment navigation.
  • Conduct IT system searches and communicate with clinicians and the multidisciplinary team.
  • Utilize SystmOne on a daily basis to facilitate patient care.
  • Contribute to audits and data collection to evaluate PCN services.
  • Facilitate routine health checks, phlebotomy, and other clinical tasks as required.
  • Support the clinical team in managing long-term conditions and adhering to infection prevention control policies.

Our network consists of four practices that work together to deliver high-quality patient care. Person Specification

The ideal candidate will possess excellent communication and organizational skills, with the ability to work effectively in a multidisciplinary team environment. A minimum of one year's experience working with healthcare professionals or in a related field is required. Desirable qualifications include a Personalised Care Institute training course, NVQ Level 3 in Adult Care, or additional qualifications in health or social care.
