Job Summary

eQuality PCN is seeking a skilled Clinical Care Coordinator to join our team. The successful candidate will be responsible for coordinating patient care, working closely with clinicians and other healthcare professionals to ensure seamless delivery of services.

Main Responsibilities

  • Proactively identify and support individuals with complex needs, providing coordination and navigation of healthcare services.
  • Conduct IT system searches and communicate with clinicians to ensure accurate and timely patient care.
  • Utilize SystmOne clinical system to manage patient data and coordinate care.
  • Contribute to audits and data collection to evaluate the effectiveness of PCN services.
  • Facilitate routine health checks, phlebotomy, and other clinical tasks as required.
  • Support the clinical team in managing long-term conditions and providing patient education.
