Job Title: Clinical Care Coordinator

We are seeking a highly skilled and dedicated Clinical Care Coordinator to join our team at Primary Care Careers. As a Clinical Care Coordinator, you will play a vital role in supporting individuals with complex health needs, providing coordination and navigation of healthcare services, and promoting health and wellbeing.

About the Role

The successful candidate will be responsible for:

  • Proactively identifying and supporting individuals with complex health needs, including the frail, elderly, and those with long-term conditions.
  • Coordinating and navigating healthcare services, including IT system searches and communication with clinicians and multidisciplinary teams.
  • Using SystmOne on a daily basis to support patient care.
  • Contributing to audits and data collection to evaluate the effectiveness of PCN services.
  • Facilitating routine health checks, including blood pressure monitoring, phlebotomy, and BMI assessments.
  • Undertaking clinical tasks as required and supporting the clinical team with the management of long-term conditions.
  • Adhering to infection prevention control policies and providing advice on diet, lifestyle, and wellbeing.

Person Specification

The ideal candidate will have:

  • Excellent communication and organisational skills.
  • Experience of working in healthcare and social care settings, including direct contact with people, families, or carers.
  • Ability to self-direct tasks and use own initiative to meet overall goals.
  • Experience of data collection and using tools to measure the impact of services.
  • Experience of working within multi-professional team environments.
  • Experience or training in personalised care and support planning.
  • Working with elderly or vulnerable people.
  • Experience of healthcare and complying with best practice and relevant legislation.
  • Supporting people, their families, and carers in a related role.
  • Minimum of 1 year experience of working with healthcare professionals and/or previous experience in the NHS or social care or relevant field.
