Job Summary

The Clinical Care Coordinator will play a vital role in supporting our patients with complex needs, providing coordination and navigation of healthcare services. This is an exciting opportunity for a motivated and empathetic individual to make a real difference in the lives of our patients.

Main Responsibilities

  • Proactively identify and support patients with complex needs, providing coordination and navigation of healthcare services.
  • Conduct IT system searches and communicate with healthcare professionals to ensure seamless care delivery.
  • Utilize SystmOne clinical system to manage patient data and coordinate care.
  • Contribute to audits and data collection to evaluate the effectiveness of our services.
  • Facilitate routine health checks, including blood pressure monitoring, phlebotomy, and BMI assessments.
  • Undertake clinical tasks as required, working closely with our multidisciplinary team.
  • Support the clinical team in managing long-term conditions and providing personalized care plans.
  • Adhere to infection prevention control policies and provide education to patients on healthy lifestyle choices.

Requirements
