Medics Primary Care Network is pleased to announce an opportunity for a Patient Care Facilitator, on a full-time basis, dedicated to delivering exceptional patient support to a diverse population. As a vital member of a multi-disciplinary team (MDT), you will play a key role in orchestrating a seamless, professional, and adaptable experience for our patients. Your approach will be holistic, ensuring that all aspects of a patient's care and support needs are addressed with a personalized touch.

You will assist the Medics integrated team in managing all essential activities, including access to services, guidance, and information, ensuring that health and care planning is timely, efficient, and centered around the patient.

Key Responsibilities:

  • Coordinate and navigate patients through the health and care systems.
  • Facilitate collaborative efforts across organizations and MDTs.
  • Make referrals to various services and health care professionals.
  • Assist patients in preparation for and follow-up of appointments.
  • Support patients in scheduling their appointments.
  • Direct patients to additional resources and information.
  • Collaborate with MDT colleagues, including social prescribing link workers and health and wellbeing coaches.

About the Candidate

The ideal candidate will possess excellent communication skills and the ability to establish effective monitoring and quality assurance systems.

Preferred qualifications and skills include:

  • Proficient IT skills.
  • Experience in a GP practice or Primary Care Network is advantageous, with knowledge of SystmOne.
  • Ability to prioritize tasks effectively.
  • Strong listening and communication abilities.
  • Passion for helping others and making a positive impact.
  • Willingness to work within a proactive and supportive team while also being capable of working independently.
  • Flexibility to support the Medics PCN multidisciplinary team as required.
