Main Responsibilities

The successful candidate will be tasked with delivering a high-quality pharmacy service to designated clinical areas. Key responsibilities include:

  • Conducting timely and precise medicines reconciliation upon patient admission.
  • Monitoring and reviewing inpatient medication in line with established acuity and complexity standards.
  • Optimizing medication regimens to ensure safe and effective discharge processes for patients.
  • Actively participating in organizational initiatives focused on medication safety and risk management, including audits and incident reporting.
  • Collaborating with the multidisciplinary team to promote the safe and effective use of medications.

Qualifications and Experience

To be considered for this role, candidates should possess:

  • A vocational Master's degree (or equivalent) in Pharmacy.
  • A post-graduate qualification in Clinical Pharmacy.
  • Completion of one year of pre-registration training and successful completion of the pre-registration examination.
  • Membership with the General Pharmaceutical Council (GPhC).
  • Job-related postgraduate qualifications or equivalent vocational experience.
