Role Summary:

The Client Care Coordinator plays a pivotal role in guiding clients through their treatment journey at Orri. This position ensures that clients experience seamless care by managing all facets of their treatment in collaboration with the multidisciplinary team (MDT) and serves as the primary contact for clients, their support networks, and external professionals. The Coordinator assists clients from the point of admission through to discharge, providing consistent support throughout their stay.

Key Responsibilities:

  • Welcome new clients and their support persons upon admission.
  • Facilitate comprehensive inductions conducted by the clinical team for new clients.
  • Verify and maintain accurate client information, including contact details and medical history.
  • Organize bi-monthly Client Review Meetings to evaluate treatment progress and plan future care.
  • Actively listen to client feedback during reviews and communicate relevant insights to the care team, ensuring all discussions are documented in the clinical records.
  • Participate in MDT reviews to gather feedback on treatment strategies and relay this information during Client Review Meetings.
  • Ensure that all clients have an active treatment plan and that review meetings are scheduled appropriately.
  • Coordinate discharge meetings, ensuring all necessary paperwork is completed and that all relevant parties are involved.
  • Finalize discharge documentation on the day of the client's discharge.
  • Attend weekly meetings to discuss any issues related to client caseloads.
  • Ensure clients and their support persons are informed about the financial aspects of treatment and the terms of service.
  • Serve as the primary contact for inquiries regarding treatment plan modifications, ensuring clarity and understanding of terms and conditions.
  • For clients utilizing Private Medical Insurance, ensure timely reviews and applications to prevent treatment interruptions.
  • Encourage client participation in outcome assessments and summarize trends during review meetings.
  • Address and resolve client concerns promptly, escalating issues as necessary.
  • Maintain clear boundaries regarding treatment requirements and communicate any changes with the clinical team.
  • Conduct monthly audits of caseload documentation to ensure compliance.
  • Analyze trends and themes quarterly to contribute to service enhancement.
