About the Role

Orri is seeking a highly skilled and compassionate Case Manager to join our team. As a key member of our care team, you will play a vital role in ensuring our clients receive exceptional support and care throughout their treatment journey.

Key Responsibilities
  • Client Onboarding: Welcome new clients and their families, ensuring a smooth transition into our care.
  • Treatment Planning: Collaborate with our clinical team to develop and implement personalized treatment plans, ensuring clients receive the best possible care.
  • Client Reviews: Conduct regular reviews with clients to discuss their progress, address concerns, and make adjustments to their treatment plans as needed.
  • Communication: Serve as a single point of contact for clients, their families, and external stakeholders, providing clear and compassionate communication throughout the treatment process.
  • Documentation: Maintain accurate and up-to-date records of client interactions, treatment plans, and progress.
  • Team Collaboration: Work closely with our multidisciplinary team to ensure seamless care coordination and effective communication.
  • Client Support: Provide emotional support and guidance to clients, helping them navigate their treatment journey and address any challenges they may face.
  • Administrative Tasks: Manage administrative tasks, such as scheduling appointments, coordinating with external providers, and maintaining accurate records.
Requirements
  • Experience: Proven experience working in a clinical environment, preferably in a case management or client care role.
  • Skills: Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and external stakeholders. Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Education: Relevant qualifications in a field such as social work, nursing, or psychology.
