The successful candidate will work closely with our partner organisations to provide an integrated service to adults with diabetes in South and West Hertfordshire.
Main Responsibilities:
- Deliver high-quality care to patients with diabetes in community settings
- Provide education and support to patients and their families
- Work collaboratively with multidisciplinary teams to achieve optimal patient outcomes

Requirements:
- Registered nurse with postgraduate experience in diabetes
