Diabetes Specialist Nurse Opportunity in Community Setting

About the Role
This is an exciting opportunity for a motivated and dynamic Diabetes Specialist Nurse to join the community team at Herts Valleys Integrated Diabetes Service (HIDS) in South and West Hertfordshire.
The successful candidate will work as part of the wider HID Service, which is an integrated service working across community and acute settings.
Hertfordshire Community NHS Trust works closely with partner organisations to provide an integrated service to adults with diabetes in the area.
Main Responsibilities
- Providing clinics and group education sessions for adults with diabetes
- Working as part of a multidisciplinary team to deliver high-quality care
- Developing and maintaining strong relationships with patients, families, and carers
Requirements
- Registered nurse with postgraduate experience in diabetes care
- Excellent communication and organisational skills
- Ability to work independently and as part of a team
