Are you interested in working with adults with diabetes in the community?
This role will be based in a community setting.
An exciting opportunity has arisen for a motivated and dynamic Diabetes Specialist Nurse to join the community team at Herts Valleys Integrated Diabetes Service (HIDS) in South and West Hertfordshire.
The role includes clinics (face-to-face or telephone) and group education sessions.
The successful candidate will work as part of the wider HID Service, which is an integrated service working across community and acute settings.
Hertfordshire Community NHS Trust works closely with partner organisations to provide an integrated service to adults with diabetes in South and West Hertfordshire.
Main Duties of the Job
We are committed to delivering a quality service, value staff training and CPD, participate regularly in student training, and have a robust appraisal and supervisory framework.
Electronic records are used throughout the Trust, further aiding our ability to work across a wide variety of settings and be in a position to work as part of a multidisciplinary team.

- Registered nurse
- Postgraduate experience working within Diabetes
- Experience of providing education
- Excellent communication skills
- Excellent organisation and planning skills
- Good IT skills

- Independent prescriber
