About This Role

We are seeking a dedicated and compassionate Health Care Support Worker to join our Heart Failure Team in Bradford. This is a fixed-term position offering an exciting opportunity to develop your skills within a specialized community setting.

As a key member of the team, you will play a vital role in delivering high-quality care to patients living with heart failure. You will work closely with our experienced nurses and clinicians to provide support, monitoring, and education to patients in their homes and clinics.

Your Responsibilities:
  • Assist the Heart Failure Team in reviewing patient cases and providing ongoing support.
  • Monitor vital signs such as blood pressure, pulse, oxygen saturation, and accurately report findings to the team.
  • Conduct patient reviews in both home and clinic settings, ensuring a comfortable and supportive environment.
  • Obtain blood samples for testing, adhering to strict safety protocols.
  • Maintain accurate patient records using our electronic health record system (SystmOne).
About You:

We are looking for a highly motivated individual with excellent communication and interpersonal skills. You will be a team player, able to work independently and effectively under the guidance of senior healthcare professionals.

Previous experience in a hospital or community setting is essential, along with a strong understanding of patient care principles. A commitment to ongoing learning and professional development is highly valued.

Essential Requirements:
  • Minimum 2 years of recent experience in a healthcare environment (hospital or community).
  • Proven ability to work effectively as part of a team.
  • Excellent communication and interpersonal skills, interacting confidently with patients, families, and colleagues.
  • Ability to work independently and use initiative while adhering to established guidelines.
  • Strong documentation skills and attention to detail.
  • A flexible and adaptable approach to meet the evolving needs of the service.
  • NVQ Level 3 in Healthcare or equivalent qualification.
  • Basic Life Support certification.
  • Valid driver's license and access to a reliable vehicle for work purposes.
