Community Health Nurse

An exciting opportunity has arisen to join the District Nursing team in Northumberland for a full-time Community Staff Nurse to cover a maternity leave.
About the RoleAs a Community Staff Nurse, you will work autonomously and as part of a team, reporting directly to your line manager, the Community Nursing Team Lead. Your key responsibilities will include assessing, planning, implementing, and evaluating care for a diverse group of patients with a wide range of conditions and health needs.
Main Duties- Assessing and planning care for patients with complex needs
- Delivering high-quality patient care in patients' homes, residential care settings, and clinics
- Developing and delivering programs of health and social care and support to patients with terminal illnesses and their families
- Performing skilled nursing care, including wound management, palliative care, and catheterization
- Interpreting diagnostic test results and communicating them to patients and carers

Person Specification- State Registration on Part 1 of the 3-part NMC register
- Additional clinical qualifications or equivalent derived through experience at diploma/degree level
- Experience of working without direct supervision in a healthcare environment
- Knowledge of or experience in coaching and mentoring practices and tools
