Main Duties- Assessing and planning care for patients with complex needs
- Delivering high-quality care to patients in their own homes, residential care settings, and clinics
- Working collaboratively with other healthcare professionals to ensure seamless care delivery
- Developing and delivering programs of health and social care to patients with terminal illnesses and their families
- Performing skilled nursing care, including wound management, palliative care, and catheterization
- Interpreting diagnostic test results and communicating them to patients and carers
