Community Learning Disability Nurse

About the Role
Southern Health NHS Foundation Trust is seeking a dedicated and compassionate Learning Disability Staff Nurse to join our North and West Hampshire Community Learning Disability Service. As a key member of our multi-disciplinary team, you will play a vital role in supporting the healthcare needs of individuals with learning disabilities in our community.
Main Responsibilities
- Assess, plan, implement, and evaluate the nursing care needs of patients with learning disabilities.
- Work under the direction and clinical supervision of a nominated preceptor during a period of preceptorship.
- Supervise, mentor, and teach qualified and unqualified staff, including students.
- Coordinate care for a specified group of patients.
- Contribute to effective multidisciplinary working within the Learning Disability team and build effective liaison with community, primary, and secondary services.
