About the Role

Southern Health NHS Foundation Trust is seeking a dedicated and compassionate Learning Disability Staff Nurse to join our North and West Hampshire Community Learning Disability Service. As a key member of our multi-disciplinary team, you will play a vital role in supporting the healthcare needs of individuals with learning disabilities in our community.

Main Responsibilities

  • Assess, plan, implement, and evaluate the nursing care needs of patients, working within the framework of MDT principles, the holistic model of care, and the six areas of clinical practice.
  • Supervise, mentor, and teach qualified and unqualified staff, including students, to ensure high-quality care and professional development.
  • Coordinate care for a specified group of patients, working collaboratively with community, primary, secondary, and tertiary services, and agencies to meet individual needs and service development.
  • Manage a caseload of patients under indirect supervision, ensuring accountability for assessing, interpreting, planning, and implementing nursing treatment and care within professional guidelines.
  • Contribute to effective multidisciplinary working within the Learning Disability team, building strong relationships with community, primary, secondary, and tertiary services, and agencies to meet individual needs and service development.
