Job Description

Job Summary:

We are seeking a highly motivated and enthusiastic Community Health Care Assistant to join our team in Chester. As a Community Health Care Assistant, you will play a vital role in delivering high-quality nursing and phlebotomy services to our patients in their own homes or in long-term care settings.

Main Responsibilities:

  • Deliver nursing services using a person-centered approach, providing quality care to patients in their own homes or in long-term care settings.
  • Work collaboratively with our integrated team to provide a range of healthcare services, including nursing, phlebotomy, and other related tasks.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Contribute to the development and implementation of care plans, ensuring that patients receive the best possible care.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in community health care.

Requirements:

  • Appropriate related qualification or willingness to complete an apprenticeship in a subject relevant to the Clinical Support Worker role.
  • Completion of or working towards an apprenticeship and gaining qualification equivalent to level 3 or above in a subject relevant to the role of Healthcare Assistant.
  • Understanding of the need for confidentiality, empathy, and sensitivity in a healthcare setting.
  • Experience of working collaboratively with colleagues, wider teams, and service users.
  • Supporting others in a care setting, whether in a professional or lived experience.
