Job Summary

BAYADA Home Health Care is seeking a skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes.

Key Responsibilities
  • Assess and Address Social and Emotional Factors: Work with our team to understand the social and emotional factors related to our clients' health problems.
  • Identify Community Resources: Identify and refer clients to community resources that address their practical and environmental needs.
  • Advocate for Clients: Act as an advocate for our clients and their families, navigating them through the community system.
  • Develop Care Plans: Participate in the development of comprehensive care plans with our multidisciplinary team.
  • Contract with Agencies: Develop and maintain contracts with public and private agencies to provide resources for our clients.
  • Education and Instruction: Educate and instruct patients, family members, and caregivers on community resources to promote healthy living.
Requirements
  • Licensure: Must be licensed by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
  • Education: Minimum of a Master's in Social Work, with at least 2 years of community-based experience.
  • Accreditation: Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
  • Home Care Experience: Home care experience a plus.
  • Communication Skills: Demonstrated ability to read, write, and effectively communicate in English.
  • Computer Skills: Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
  • Travel: Ability to travel to cases, as assigned.
