We are seeking a skilled Mental Health Practitioner to join our Hospital Discharge Team at the Herefordshire and Worcester NHS Trust.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans to support service users and their families/carers.
- Provide evidence-based interventions and specialist advice to promote recovery and well-being.
- Work collaboratively with a multidisciplinary team to enhance the unique value of mental health practitioners and promote a culture of excellence.
- Deliver evidence-based interventions following NICE guidelines to promote recovery and improve outcomes.
- Regularly review care and outcomes, supporting service users to achieve clinical goals and complete clinical outcome measures.

To be considered for this role, you must be a skilled Mental Health Practitioner with experience working in a Hospital Discharge Team setting. You must also be able to work collaboratively with a multidisciplinary team and provide evidence-based interventions to promote recovery and well-being.
