About the Role

We are seeking a highly skilled and dedicated Mental Health Nurse to join our Care Home In-Reach Team. As a key member of our multidisciplinary team, you will provide high-quality care and support to individuals with dementia and mental health needs.

Key Responsibilities
  • Manage a caseload of patients to ensure timely assessment, diagnosis, and treatment
  • Monitor treatments and make adjustments as necessary
  • Develop and implement effective discharge plans
  • Conduct physical health observations and take appropriate action
  • Monitor and evaluate treatment effectiveness
  • Provide advice on dementia management and healthy lifestyles
  • Carry out complex risk assessments
  • Report and escalate incidents and safeguarding concerns
  • Participate in multidisciplinary meetings and provide leadership to junior staff
