Position Overview

Hedena Health Ltd is seeking a dedicated Clinical Care Coordination Specialist to join our dynamic Contracts and Clinical Pharmacy Team. This role encompasses a diverse range of responsibilities, including the daily management of repeat prescriptions, addressing medication inquiries from patients and external entities, conducting prescribing audits, responding to safety alerts, overseeing the medication recall system, entering clinical data, and engaging directly with patients to promote service utilization.

In this position, you will play a vital role in proactively identifying and collaborating with individuals, particularly the elderly and those with chronic conditions, as well as conducting outreach for patients who may not be actively using our services.

Key Responsibilities

The Clinical Care Coordination Specialist will provide essential clinical administrative support to our Contracts and Clinical Pharmacy Team. Responsibilities include:

  • Processing prescription orders within the clinical system.
  • Responding to medication-related inquiries from patients, staff, community pharmacies, and care facilities.
  • Assisting clinicians in managing documentation workflows related to prescriptions.
  • Promoting electronic repeat dispensing and online ordering systems.
  • Collaborating with prescribers to ensure a responsive repeat prescription service.
  • Liaising with patients and healthcare providers to secure accurate prescription information.
  • Managing prescription requests from vulnerable and high-priority patients.
  • Conducting prescribing audits and addressing medication safety alerts.
  • Utilizing population health intelligence to identify patient cohorts and track intervention outcomes.
  • Coordinating and providing administrative support for multidisciplinary team meetings.

Strong communication skills and a high level of IT proficiency are essential, with familiarity in systems such as EMIS and Docman 10 being advantageous.
