Position Overview

Hedena Health Ltd is seeking a dedicated Clinical Care Coordination Specialist to join our dynamic Contracts and Clinical Pharmacy Team. This role is essential in providing administrative support and will encompass a variety of responsibilities, including the management of repeat prescriptions, addressing medication inquiries from patients and external organizations, conducting prescribing audits, implementing safety alerts, overseeing the medication recall system, entering clinical data, and engaging directly with patients to promote service uptake.

The successful candidate will play a pivotal role in proactively identifying and collaborating with individuals, particularly the elderly and those with chronic conditions, as well as reaching out to patients who may not be actively utilizing our services.

Key Responsibilities

The Clinical Care Coordination Specialist will be responsible for:

  • Providing comprehensive clinical administrative support to the Contracts and Clinical Pharmacy Team.
  • Managing daily operations related to repeat medication processing and responding to medication-related queries.
  • Conducting audits to ensure safe prescribing practices and actioning relevant safety alerts.
  • Maintaining the medication recall system and ensuring accurate clinical data entry.
  • Working closely with clinical teams, Clinical Pharmacists, and multi-disciplinary teams to facilitate patient navigation across health and care services.

The ideal candidate will demonstrate a compassionate approach, possess a solid understanding of healthcare practices and medications, and provide exceptional customer service. Strong communication skills and a high level of IT proficiency are essential, with familiarity in systems such as EMIS and Docman 10 being advantageous.
