Job Overview

Hedena Health Ltd is seeking a dedicated Clinical Care Coordination Specialist to enhance our Contracts and Clinical Pharmacy Team. This multifaceted role involves the daily management of repeat prescriptions, addressing medication inquiries from patients and external organizations, conducting prescribing audits, responding to safety alerts, overseeing the medication recall system, entering clinical data, and engaging directly with patients to promote service utilization.

In this position, you will play a vital role in proactively identifying and collaborating with individuals, particularly the elderly and those with chronic conditions, as well as conducting outreach to patients who may not be utilizing our services.

The ideal candidate will possess experience in community or hospital pharmacy settings or within General Practice, although training will be provided for the right individual.

Key Responsibilities

As a Clinical Care Coordination Specialist, your responsibilities will include:

  • Providing clinical administrative support to the Contracts and Clinical Pharmacy Team.
  • Managing the daily processing of repeat prescriptions and addressing medication-related queries.
  • Conducting prescribing audits and implementing safety alerts.
  • Overseeing the medication recall system and ensuring accurate clinical data entry.
  • Engaging with patients to encourage the uptake of services and support their health needs.
  • Collaborating closely with clinical teams, Clinical Pharmacists, and other members of the multi-disciplinary teams to ensure seamless patient navigation across health and care services.

The successful candidate will demonstrate a compassionate approach, possess a strong understanding of healthcare practices and medications, and deliver exceptional customer service. Excellent communication skills and a high level of IT proficiency are essential, with familiarity in systems such as EMIS and Docman 10 being advantageous.

Qualifications

  • GCSE and A-Level Qualified.

Experience

  • Experience in the NHS with a solid understanding of the primary care sector and clinical environments.
  • Strong literacy and numeracy skills with a keen attention to detail.
  • Excellent verbal and written communication skills, capable of conveying complex information clearly.
  • High level of IT skills, particularly in Microsoft Office applications.
  • Proven ability to work collaboratively and build strong relationships within a team.

Desirable

  • Familiarity with healthcare improvement methods and experience in project development.
  • A full UK driving license.
