Main Responsibilities:

The successful applicant will independently provide nursing care to the Medical Treatment Facility (MTF) population, managing an individual case load and utilizing consultations to identify, assess, and diagnose patients with complex health needs, minor injuries, minor illnesses, and specific diseases within their competency.

Job Responsibilities:

Your duties will encompass, but are not limited to:

  • Delivering safe, evidence-based, cost-effective, and personalized patient care.
  • Overseeing the management of patients with chronic conditions such as Asthma, Diabetes, Chronic Heart Disease (CHD), and hypertension, ensuring the maintenance of the Chronic Disease Register.
  • Performing technical nursing treatments and procedures for patients with complex healthcare needs (e.g., wound care, sutures removal, Electrocardiograms (ECGs), Cytology, Audiometry, Ear Care).
  • Coordinating the development of health plans tailored to meet patients' needs, including diagnostic tests, treatment plans, health promotion, and preventive measures.
  • Facilitating the development and delivery of specialized services, such as long-term disease management, acting as a regional resource for advice and regional clinics.

Qualifications:

Essential:

  • Professional and specialist knowledge in primary healthcare, acquired through a postgraduate degree/diploma or equivalent experience in clinical assessment and diagnosis.
  • Learning and Assessing in Practice Qualification or equivalent training.
  • Quality improvement methodology or willingness to undertake training.
  • Current certifications in Basic Life Support (BLS), Anaphylaxis, Automated External Defibrillation (AED), Infection Prevention and Control (IPC), and Safeguarding.
  • Registered non-medical prescriber or willingness to undertake training.

Experience:

Essential:

  • Proficiency in relevant IT systems, subject to appropriate training.
  • Experience in history taking, physical assessment, and the triage and treatment of minor injuries/illnesses.
  • Experience in leading health and well-being clinics.
  • Experience in providing care to patients with chronic illnesses and mentoring other staff.
  • Evidence of continuous professional development and practical application of new skills.

Registration Details:

Essential:

  • Valid membership registration number with the relevant professional body.
  • Current registration with the Nursing and Midwifery Council.
