Care Coordinator

We are seeking a skilled and compassionate Care Coordinator to join our team at Cambridgeshire County Council. As a Care Coordinator, you will play a vital role in supporting individuals with complex needs, providing a personalised approach to care and connecting them with relevant services.
Key Responsibilities- Implement a new approach to support individuals who are using multiple services but require a more tailored approach to meet their needs.
- Direct work with adults in their homes or communities, focusing on non-medical needs that cannot be met by frequent Emergency Department attendances.
- Start with a What Matters to You conversation, offering a personalised care approach that responds to an individual's priorities and needs.
- Link with and bring in other support as appropriate.
We are looking for someone with a good understanding of health inequalities and the wider determinants of health. You will have significant experience as a case holding practitioner/worker, with an approach that aligns with personalised care. A coaching qualification or training in motivational interviewing, appreciative enquiry, or similar is also desirable.
