Role Overview:

As a Community Support Coordinator, you will play a crucial role in enhancing the lives of individuals who frequently utilize various unplanned services without receiving adequate support. Your focus will be on addressing non-medical needs through direct engagement with adults in their homes or community settings.

Key Responsibilities:

Initiate a personalized care strategy by conducting 'What Matters to You' conversations, ensuring that care is tailored to individual priorities and requirements. Collaborate with various services to create a cohesive support network for clients, ensuring their diverse needs are met effectively. Provide direct assistance to individuals whose health concerns cannot be resolved through regular visits to Emergency Departments.

Candidate Profile:

A solid understanding of health disparities and the broader factors influencing health outcomes is essential. Proven experience as a case management practitioner, demonstrating a commitment to personalized care approaches. Qualifications in coaching or training in motivational interviewing, appreciative inquiry, or related fields are required. Experience in collaborative working and navigating complex service landscapes is highly valued.
