About the Role
We are seeking a skilled and compassionate Care Coordinator to join our team at Cambridgeshire County Council. As a Care Coordinator, you will play a vital role in supporting individuals who require non-medical care and connecting them with relevant services.
Key Responsibilities
- Implement a new approach to support individuals who are using multiple services but require a more tailored care plan.
- Provide direct support to adults in their homes or communities, focusing on their non-medical needs and priorities.
- Conduct What Matters to You conversations to understand individuals' needs and priorities, and develop personalized care plans accordingly.
- Link with other support services and professionals to ensure a comprehensive care approach.
About You
To be successful in this role, you will need:
- A good understanding of health inequalities and the wider determinants of health.
- Significant experience as a case holding practitioner/worker, with a focus on personalized care.
- A coaching qualification or training in motivational interviewing, appreciative enquiry, or similar.
- Experience of partnership working and navigating services.
