Join Our Team as a Band 5 Community Nurse

We are seeking a highly skilled and compassionate Band 5 Community Nurse to join our team at HCRG Care Group. As a Community Nurse, you will play a vital role in delivering high-quality care to our patients in their own homes.

About the Role

As a Band 5 Community Nurse, you will be responsible for contributing to the holistic assessment, planning, delivery, and evaluation of care to patients within their own homes. You will develop and implement programmes of care, administer medication, and work closely with other healthcare professionals to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Conduct holistic assessments of patients to identify their care needs
  • Develop and implement individualised care plans to meet the needs of patients
  • Administer medication, including controlled drugs, and provide wound care and IV therapy
  • Work closely with other healthcare professionals to ensure seamless care delivery
  • Provide education and support to patients and their families to promote independence and well-being

Requirements
  • NMC registration as a Registered Nurse
  • Full UK driving license and access to a car for work purposes
