About the Role
The Old School Surgery is seeking a highly motivated and organized Care Navigator to join our team. As a Care Navigator, you will be the first point of contact for our patients, providing support and guidance on choosing the right service at the right time.
Key Responsibilities
- Maintain and monitor the practice appointment system.
- Identify and book appointments and home visits in line with practice protocols.
- Process personal, telephone and e-requests for appointments.
- Provide exceptional customer service to patients, responding to their queries and concerns in a professional and courteous manner.
- Work collaboratively with the clinical team to ensure seamless communication and coordination of patient care.
- Monitor and maintain the reception area, ensuring a clean and welcoming environment for patients.
- Support all clinical staff with general tasks as requested.
Requirements
- Educated to GCSE level or equivalent.
- Previous experience of working with people.
- Previous experience of working in Primary Care (desirable).
