We are seeking a dedicated and compassionate individual to join our team as a Positive Intervention Practitioner/Social Care Worker. As a key member of our care team, you will play a vital role in providing high-quality care and support to our residents with learning disabilities and mental health conditions.
About the Role- Provide personal care, including bathing, toileting, dressing, and help with feeding
- Follow care plan guidance and company policies and procedures
- Attend and complete training as required
- Work collaboratively with the care team to develop and implement individualized care plans
- Contribute to the development of interventions aimed at reducing challenging behaviors
- Offer individual and group work to residents under the supervision of the Clinical Lead and/or Senior Practitioner
- Facilitate the smooth and efficient implementation of care plan systems within the homes
